Consent to administer medication

Wonderland School strives to provide each child with a safe and healthy environment in which to learn.  In order to assure the health and safety of all children enrolled in Wonderland School, any child who presents with a fever over 100.3 F will need to go home.  A child will have to have a temperature of less than 100.3 3 F for a minimum of 24 hours prior to returning to school.  In the event of a fever we can, with your permission, medicate your child with over-the-counter fever reducer until your arrival.

All over the counter pain relievers/fever reducers will be dosed and administered as indicated on the medication packaging.

Medical Release

I hereby authorize school personnel to send my child to a hospital to receive any emergency treatment deemed necessary.  This procedure will be employed in any instance of injury or severe illness or when school authorities feel that my child's condition warrants such action.  Further, I agree to assume the costs of such emergency care for all medical expenses incurred, including that of the EMS Ambulance Service.                                                                                          ___Yes ___No
